Application for Approval of a MAF Transitional Facility and Facility Operator

The above forms are required when applying for Transitional or Containment facility and Operator approval. These forms can be used for approvals under all MAFBNZ Facility Standards, and once completed, should be sent to the Operational Standards and Facilities Group at an address on the form.

If you wish to cancel part or all of your facility approval submit this signed form to your Biosecurity Inspector for processing

 

Before sending in your forms check the following:

For brand new facilities or facilities moving location:

You have filled out the application for approval of a facility form including all mandatory fields
You have filled out the application for approval of an operator form including all mandatory fields
The Operator has filled out the consent to disclosure of information (police check) form
The Operator has taken the Operator training course (or is booked in for training)
You have filled out the application for a credit account form (new facilities only) or provided an updated account number
You have included with the application a draft of your operating manual with a site plan
If you are importing sea containers you have enough trained Accredited Persons to check the containers
You have signed the form(s)

For new Operators at an approved facility:

You (the Operator) have filled out the application for approval of an operator form including all mandatory fields
You (the Operator) have filled out the consent to disclosure of information (police check) form
You (the Operator) have taken the Operator training course (or are booked in for training)
You have provided an updated version of the operating manual with staff changes
You have signed the form(s)
