Phyto E-Cert Application Forms

Introduction

The following forms enable an organisation and/or individual user to register to use Phyto E-cert.

Based on the reason for your request you will be required to provide organisation and/or individual user level details.

Open the application form (document link below) that suits your required action. Fill the details requested and send the completed form to plantexports@maf.govt.nz or fax to Plant Exports on 04 8940733

New Organisation Request registrations for Phyto E-cert incur a $50 fee. This is a one off fee that covers the processing of the request and is inclusive of GST. Please complete the New Organisation Request Form, print it, and send it with your cheque to Plant Exports, MAFBNZ, PO Box 2526, Wellington. Alternatively you can provide Credit Card details on the form which should be sent by fax or mail.

Application Phyto Ecert removal of Individual Access Request Use this form if you need to cancel a user’s access to Phyto E-cert Form
